Auxiliary ID Card Application Process

1. Download USCGAUX Member ID Form. "Click here for download edible word.doc"

2. Add a square shaped photo to email (READ BELOW photo instructions) with a red background. The member must be in uniform.

(IMPORTANT)  Member Facial picture 4 to 5 feet away, "Red background, No officer shoulder boards" to try to get the correct picture aspect ratio for DRAUX ID Card printer, the idea is to get an acceptable picture of yourself on the new ID card. 


3) Name the JPG file with your member number. ie , "your name .jpg"

4) You cc the email to yourself for safe keeping. 

Responsibility
The Flotilla Commander will ensure that all ID card applications/photos comply with the procedures set forth below.

The Flotilla Commander will review and forward all applications/photos to the DIRAUX District 5NR, ID card team for review. The DIRAUX , ID card team will review the applications/photos for compliance for production. Upon completion the I.D. card will be mailed directly to the member.
The Flotilla Commander may appoint a representative within the Flotilla to carry out application process.
Procedure:
Members will fill out the application form completely. The form should be neat and the information must be readable. Applicants must print the information on the form. Please use the provided link to the ID Card application form. This is the only form DIRAUX authorized for use.

The applicants must have passed the initial background check and have been approved by the Director for enrollment before the application can be processed. These are members who are IQ or BQ status.



Photos for the ID card must follow the following instructions:


Photos taken with a digital camera must be in JPEG format. If using a smart-phone, please take the picture in “  landscape square shaped photo ”
Photo will be taken with red background. Please keep clean and wrinkle free.
Members must be in uniform, either Tropical Long or ODU. No office insignias, collar devices or shoulder boards. For trops, only Member shoulder boards (plain, no stripes) are used. For ODU's, only member collar devices are used. Photo will be of head and shoulders with member facing forward.
Members will comply with dress code (ref. Chapter 10, Section C of Auxiliary Manual COMDTINST M1670.1series). Uniform must be neat and clean. Hair combed and off the collar.
Each photo must be identified by the Last Name of the member and Member I.D. number. An ID Card application with corresponding name must accompany each photo.
